A Case of Chronic Neutrophilic Leukemia

Abstract

|
Chronic neutrophilic leukemia is a rare form of myeloproliferative disorder. It is characterized by sustained neutrophilia with splenomegaly in the absence of fever, inflammation or any detectable cause for a leukemoid reaction, and laboratory
findings
show elevated neutrophil alkaline phosphatase(NAP), serum vitamin B12 and serum uric acid, and negative Philadelphia chromosome. Authors experienced a case of chronic neutrophilic leukemia in a 53-year-old man, evolving to the stage of blast
crisis. WBC
count of the peripheral blood was 68.600/¥ìL with 76% segmented neutrophils. The underlying disease for a leukemoid reaction had not been detectod. NAP score and the serum levels of vitamin B12 and uric acid were elevated. Bone marrow aspiration
revealed marked myeloid hyperplasia with 32% of mature neutrophils. Chromosomc study showed a normal karyotype without Philadelphia chromosome or rearrangement within breakpoint cluster region(bcr). Four months after the stage of remission, the
hematological picture evolved into a blast crisis of myeloid features (myeloblast 24%, promyelecyte 13%).
